• National Organization for Health Care Services Provision - E.O.P.Y.Y.
• Subsidiary Social Security and Welfare Fund for Security Officers - T.E.A.P.A.S.A.
• Unified Subsidiary Social Security and Welfare Press Organization - E.D.O.E.A.P.
• Health Fund for Customs Officers - T.A.P.E.P.A.
• Subsidiary Social Security Institution for EYDAP Personnel

For valid E.O.P.Y.Y. referrals, please note:

• Physicians' signature and stamp must be clearly legible.
• Do not mark at the foot of the referral that the examination is performed only in public institutions.
• The date of the examination must precede the expiry date of the referral.
• The signature of the examinee must be present.
